N.A.I.C., L.C.H.T. License Application

Please note: NAIC does not guarantee approval of any application for any membership. All requirements must be satisfied, including a complete application filed, all fees and dues donations paid in full, background verification check and confirmation of personal references, prerequisite coursework completed, and personal interview including ceremony with NAIC Elders, as well as any other special requirements to be determined on a case-by-case basis. Failure to complete any step or procedure will end the application process immediately. Any material misrepresentation or indications of less than stellar character or criminal activity of any kind will end the application process immediately. All donations to N.A.I.C. are non-refundable on submission as a precondition to being considered for N.A.I.C. Licensure-licentiate as per N.A.I.C. Cancellation and Refund Policy.
